USAH Forms and SafeSport Training

We will need a USAH Consent to Treat form completed for each player registered for the current season. Please access the form here and upload the completed document at the time of registration.

We will need a USAH Code of Conduct attested for each player at the time of registration for the current season. Please be sure to read this form and attest to its requirements at the time of registration.

We will need a Letter of Intent form completed for each player who is rostered on a "WAHL" or "Travel" Team. Please access the form here and upload the completed document at the time of registration.

All players who are rostered for the upcoming season are required to have a CAHC Code of Conduct and Media Release attested to. Both the player and a parent or guardian must read and attest to this form during registration for the current season.
