USAH Code of Conduct form
We will need a USAH Consent Code of Conduct form completed for each player registered for the current season. Please upload the completed document at the time of registration.
CAHC Letter of Intent for all players
We will need a Letter of Intent form completed for each player who is rostered on a "WAHL" or "Travel" Team. Please access the form here and upload the completed document at the time of registration.

We will need a CAHC Code of Conduct and Media Release for each player who is rostered. Please access the form here and upload the completed document at the time of registration.
MANDATORY Safe Sport Training for players who turn 17 years of age before December 31 of the current season
Please note, if your player is a 2006 birth year or earlier, he/she will be required to take the SafeSport Core Training or Refresher Course prior to participation for the 2023-24 season. Please access the training by click on the button below.
